Abstract:

Loss of lives and properties caused by natural or man-made disasters, such as earthquake and fire, is a common issue. However, there are some buildings, which are more vulnerable to mentioned disasters due to the excessive number of residents and stories. DORMITORIES are highly vulnerable because not only they meet the above conditions, but also residents’ including students’ and employees’ lack of preparation and awareness about the ways of facing with danger increases their vulnerability. In this paper, dormitory of married students of Shahid Beheshti University has been selected. The organizational and non-structural vulnerabilities have been detected by applying a descriptive-analytical method. Firstly, the incident command system, solutions for risk mitigation of organizational and non-structural constructions in addition to some suggestions for preparing people to face with the disasters have been presented in accordance with the reference to disaster management theories and based on the data collected from the observations, questionnaires and interviews. Different measures are proposed based on adaptation of the dormitory organizational chart to incident command system diagram such as taking maneuvers, promoting safety culture, providing emergency evacuation plan and suggesting risk mitigation solutions for non-structural elements.

Abstract:

Extended Abstract Background and Objectives: Research has shown that there is a strong connection between people’s health and the physical environment in which they work, live, or treated. Sick building syndrome is an illness caused by the building itself, leading individuals in the environment to suffer from inadequate physical and mental well-being, often resulting in symptoms such as nervousness, mucous membrane irritation, respiratory problems, and skin irritations. This situation can arise from various reasons such as cold air blast, very high or very low room temperature, heavy air, smoke and smell caused by people smoking, static electricity, poor condition of lighting in the room, unpleasant smell, dust, noise, etc. Such a situation can occur in any enclosed space where people spend a significant amount of time. Student DORMITORIES exemplify such environments, as students inhabit them for several years. Hence, this study was carried out to investigate the symptoms and reasons for this sick building syndrome, specifically within student DORMITORIES. Methods: The current investigation is a cross-sectional, descriptive-analytical study carried out over a two-month period, specifically in November and December of 2019. It focused on 325 university students residing in the men’s and women’s DORMITORIES of Shahrood University of Technology, with the exclusion of 3 special patients and 20 incomplete questionnaires from the research sample. For data collection, MM040EA standard questionnaires were administered to the university students in an online form and the required data was subsequently collected. Data was analyzed through binomial test and chi-square test. Findings: According to the surveys, fatigue (64. 24%), difficulty concentrating (58. 28%), and headaches (53. 48%) are the predominant symptoms of sick building syndrome. Additionally, significant contributing factors to this syndrome encompass noise, poor air quality, excessively high room temperatures, and dry air, with the study also highlighting the impact of neuro lighting on its prevalence. Among the various factors outlined in this article, severe weather conditions and exceptionally high room temperatures during the study period in the student dormitory stand out as the primary culprits for inadequate room ventilation. In this research, there is a significant relationship between the prevalence of building syndrome and all the factors related to the comfortable conditions of the dormitory and the physical environment and psychosocial conditions of the dormitory. Conclusion: The findings from this study suggest that the prevalence of sick building syndrome, at 80%, surpasses both the figures reported in prior research and the author’s initial expectations. This discrepancy is viewed as unfavorable and assessed as concerning. Since these symptoms are related to the physical condition of the room where people stay, it is necessary to review the conditions of students’ presence in the dormitory, to reduce this problem by reducing people’s attendance and increasing the per capita accommodation space. When it is impossible to solve the students’ dissatisfaction completely and eliminate the issue of sick building syndrome, identifying the causes of the problem instead of focusing on the symptoms is essential. Undoubtedly, the interior design of any structure holds significant importance in the overall design process. Consequently, designers and architects should enhance their consideration of interior environment design and increase their awareness of its impact on occupant health, particularly in spaces like student DORMITORIES where occupants spend a substantial amount of time. In a broader effort to reduce the prevalence of sick building syndrome in student DORMITORIES, it should be recognized that the improvement of dormitory facilities and the renovation of older structures will prove highly beneficial in addressing this issue.

Abstract:

Background: Methadone is a synthetic opioid, used in treatment of chronic pains. The current study was carried out to evaluate the QTc interval in Iranian causalities (JANBAZAN) of Iran-Iraq war receiving maintenance methadone treatment.Materials and Methods: In 2010, one hundred war causalities in Isfahan who chronically take daily dose of 20 mg or more of methadone (more than 2 weeks), and did not have the history of cerebrovascular or coronary artery diseases, cardiac pacemaker, congenital prolonged QTC, or taking drugs affecting QTc, or having electrolyte abnormalities, were selected for the study. An electrocardiogram was taken from each patient using cardiofax instrument, and QTC was calculated manually. The data was analyzed using SPSS software with descriptive statistical methods and Pearson’s correlation coefficient.Findings: All patients were male and had the mean age of 45.6 ± 6.1 years. The patients received 20-240 mg methadone daily for 1 to 108 months. The QTc was prolonged in 25% of the patients (QTc5 450 ms), with the mean of 472.72 ± 18.5 ms (range 450-508 ms) and the mean daily dose of methadone 85.2 ± 59.0 mg. No significant relationship was observed between QTC interval on the one hand, and methadone dose (R=0.025, P=0.8), duration of treatment (R=-0.048, P=0.68), age (R=0.037, P=0.71), and weight (R=0.1, P=0.21) of the patients, on the other hand. None of the patients had faint, syncope, arrhythmia, or sudden death. Conclusion: Oral methadone causes prolongation of QTC interval. However, the dosage of methadone and duration of treatment were not statistically related to QTC interval.

Abstract:

Introduction: Atypical antipsychotic drugs in comparison with typical antipsychotics produce fewer extra pyramidal symptoms at clinically effective doses. However there has also been a growing number of reports of new onset diabetes mellitus in patients receiving these drugs. The aim of this study was the comparison of diabetes mellitus incidence in mentally JANBAZAN ( Iran-Iraq war veterans) undertaken typical with atypical antipsychotics.Material & Methods: In this study the incidence of new onset DM was assessed in 200 subjects referred to Amiralmomenin clinic, Isfahan, Iran. Every patient was included in this study if he was receiving atypical or typical antipsychotic drugs or combination of both types, at least for one year. Sampling was convenient. Fasting blood sugar, triglyceride and cholesterol before taking the drugs was documented in their records. New onset DM was defined based on 126 mg/dl or more for FBS which was confirmed with GTT. For comparison of FBS mean pre and post intervention, NIDDM incidence, the relation of background variables with NIDDM, and the relation of NIDDM incidence with dosage and duration of drug therapy, paired t-test, chi-square, logistic regression and ANOVA were used. Results: Subjects were divided into three groups based on the type of drugs used 135 patients using typical antipsychotics (Chlorpromazine, Perphenazine, Haloperidol, …), 39 patients using atypical antipsychotics (Clozapine & Risperidon) and 26 patients receiving combination of drugs. It was found that DM developed following treatment in three groups were 12/8%, 6/7% and 3.8% respectively. There was no significant differences between three groups regarding the incidence of new onset DM.Conclusion: Findings showed that, patients treated with antipsychotic drugs had a moderately increased risk of DM especially with atypical antipsychotics. It is suggested to evaluate every patient before treatment for risk factors of DM.
